Opt-in/lead form

The Diary Of A CEO Opt-in/lead form — “Where should we send your guide?”

Why it works

“Send me the guide” opens a Klaviyo modal with one field. Email only, no name, so the cost of saying yes is as low as it gets. The questions come on the next two screens, after the address is already saved.

  1. The Diary Of A CEO — “Where should we send your guide?” (screen 1)
One field and “Unlock free guide”. The button copy treats the email as a key, not a sign-up.
  2. The Diary Of A CEO — “Where should we send your guide?” (screen 2)
Filled with a tagged address and submitted. Klaviyo saves each step as it goes, so the email was captured here even though the later screens were not finished.
