A video thumbnail ("add 30-50 new members in 90 days") sits above a "Grab the Free Training" form, whose fields include a three-way delivery preference. Like the Playbook page it carries a hidden second form with shipping and billing fields, posting to /double-v-copy. The form was not submitted.
